| 目的
通过描述医联体模式建设初期社区医院与核心医院间药品对接情况,探讨社区医院药品种类与慢病患者管理需求间的差别,探索进一步完善医联体内部单位间药品对接的关键点。
方法 将北京地区以北京大学第三医院为核心的的海淀区中东部医疗联合体下属的21家一级社区医院作为研究对象,探讨区属社区医院和大学院校医院在高血压、糖尿病、冠心病、脑血管病4种疾病类型所用药品中的对接达标情况。结果 北医三院医联体成员单位中,4类重要慢性疾病药品上下级医疗机构对接达标率较低(药品对接达到或超过60%视为达标)。区属社区医院和大学院校医院在4类疾病组中的达标率间存在较大差异,其中高血压达标率在区属社区医院为85.7%,而在大学院校医院仅为21.4%,组间差异有统计学意义(P=0.016)。结论 需要加强管理社区医院慢病药品对接率,冠心病、脑血管疾病及大学院校校医院应为关注重点。 |

| Analysis of Drug Docking Situation of Chronic Diseases in Medical Alliance |

Objective
To describe the drug docking situation between community hospital and the core hospital at the early stage of construction of medical alliance, to discuss the differences between community hospital drug types and chronic disease management demand, and to explore the key point to further improve the medical alliance drug docking. Methods 21 first-level community hospitals in the Middle East Medical Alliance of Haidian District of Beijing,in which the Peking University Third Hospital was the core hospital,were selected as the research objects. The docking compliance rates on drug supplies in 4 diseases of hypertension,diabetes,coronary heart disease and cerebrovascular disease, were analyzed within sub-community hospitals and college-hospitals. Results In the hospitals of medical alliance, docking compliance rate of on drug supplies in the four major chronic diseases were low (compliance on or over 60% drugs). There were significant differences in the rate of compliance among the four major chronic diseases as comparing the data between of sub-community hospitals and of college-hospitals. Among them,the compliance rate in hypertension in sub-community hospitals was 85.7%,while in college-hospitals was only 21.4%. The difference was statistically significant (P=0.016). Conclusion It’s essential to strengthen the management of community hospital on chronic disease and enhance the drug docking rate. Coronary heart disease,cerebrovascular disease and university hospital should be the focus of attention. |
