| 目的 分析医疗机构间医生协作意愿，探讨完善分级诊疗制度的可行措施，为形成科学、合理的就医秩序提供参考依据。方法 随机抽取南京市高淳区人民医院主要科室共120名医生进行问卷调查。调查内容包括调查对象的基本情况、分级诊疗过程中医院与社区间医生协作意愿以及加强监管的建议。结果93.7%和65.8%的医生分别认为需要建立社区首诊制度和提高社区医疗水平，65.8%和69.4%的医生分别希望通过分诊信息系统进行沟通和协作，58.56%的医生认为完善分级诊疗需合理调整医疗机构间的经济利益，54.95%和68.49%的医生分别认为政府应加强监管和改革配套医疗保险制度。结论 分级诊疗制度下医疗机构间医生对协作的意愿明确，健全社区首诊制、提高社区卫生服务机构医疗水平，加强沟通协作、建立分诊机构间利益协调机制，加强监管、完善医保制度配套措施是完善分级诊疗的有效措施。 |

Objective
To analyze the investigation of doctors’ collaboration willingness among medical institutions，and to explore the feasible measures to provide references to form a scientific and reasonable treatment order for improving hierarchical medical system. Methods 120 doctors from different departments in the People’s Hospital of Gaochun District, Nanjing City were chosen by simple random sampling to fill questionnaires, including the basic conditions，doctors’ willingness to communicate and collaborate among hospitals and community health institutions and suggestions to strengthen the supervision of hierarchical medical system. Results 93.7% and 65.8% doctors said that it’s necessary to establish first-treatment system and raise the health care level. 65.8% and 69.4% doctors hoped to strengthen communication and collaboration by referral information system，58.56% doctors thought that it is necessary to adjust the economic interests among medical institutions，and 54.95% and 68.49% doctors thought that governments should strengthen the supervision and form a complete set of medical insurance system. Conclusion Doctors’ collaboration willingness among medical institutions under hierarchical medical system is strong. Improving first-treatment system and the health care level at community health service institutions，establishing economic benefit coordination mechanism and strengthening communication and collaboration，improving supervision and forming a complete set of medical insurance system were the effective measures to perfect the hierarchical medical system. |
