目的 调查二、三级医院手卫生现状，为医院手卫生改善提供参考方向。方法 采用世界卫生组织手卫生自我评估框架对黑龙江10家医院（二、三级医院各5家）进行问卷调查。结果 (1)两级医院在手卫生总体水平及各维度评分差异均有统计学意义（P＜0.0001）；(2)手卫生实施效果方面差异有统计学意义，手卫生认知程度与主观感知重要性间存在相关性；(3)不同科室、岗位医务人员在手卫生依从率方面差异均有统计学意义（P＜0.05）。结论 两级医院手卫生整体水平不高，二、三级医院分别处于基本和中等水平，需采取有效措施提高手卫生水平，建议在手卫生制度完善、评估与绩效、工作地点提醒、安全氛围等方面加强。 |

Objective To investigate the status of hand hygiene in Grade II and III hospitals，and to suggest reference direction for the improvements of hospital hand hygiene. Method Questionnaire surveys based on HHSAF of WHO were conducted in 10 hospitals (5 for Grade II and III，respectively) in Heilongjiang. Result There were statistically significant differences between the hospitals with different grades in multi-aspect rating of hand hygiene. There were also significant differences observed in the implementation effects of hand hygiene. Correlations were found between awareness and perceptual importance of hand hygiene. Moreover，significant differences were also observed in the overall compliance rates of hand hygiene from medical staff in different departments and positions in individual hospitals. Conclusion The overall rating of hand hygiene in the hospitals with Grade II and III are relatively low. The Grade II and III hospitals involved in this study are ranked as basic and medium level respectively. It is suggested to take effective measures to improve the overall level of hand hygiene，such as System Improvement，Training and Education，Evaluation and Feedback，Workplace Reminders，strengthening of Safety Climate and so on. |
