| The paper reviews the development of continue nursing specialist in the hospital and the results, describes the hospital continue nursing service department through the establishment of the architecture. The hospital takes telephone follow-up, provides telephone consultation, and implements psychological rehabilitation team activities, family visits and community referral to supply continuing nursing care for discharged patients in a variety of ways. In this way, the hospital meets the different needs of discharged patients and ensures the quality of life after discharge. |
