目的
对基本药物制度的实施效果进行综合分析和评价。方法 运用因子分析方法探索影响基本药物实施效果的潜在评价结构维度，筛选关键评价指标。结果 通过探索性因子分析方法提取了影响基药实施效果的三大公共因子：分别为药物使用合理性因子、基层医疗机构收入构成因子、居民疾病经济负担性因子。基本药物制度实施后，重庆市乡镇医院药物使用合理性指标有了较大的改善，抗生素的使用率明显下降，全部实现了WHO针对发展中国家推荐的最低使用标准。基药实施后药占比波动在32.12%～43.33%之间，财政投入在30.22%～35.16%之间。结论 基药实施后，药物使用合理性因子有了明显改善，基层医疗机构收入结构因子也逐步趋近合理，居民可负担性因子改善幅度较小，建议进一步加大政策投入、完善相关费用控制措施以及医改监测指标。 |

Objective
To analyze and evaluate the implementation effect of the basic drug system. Method Using the factor analysis method to explore the potential structure dimension of evaluation of influence of essential medicines effect, screening the key evaluation index. Results We extracted the three common factors through exploratory factor analysis method: rationality drug use factor, income structure in basic medical institutions factor, residents’ economic burden of disease factor. After the implementation of the essential medicines system, the indicators of rationality drug use have greatly improved in township hospital in Chongqing, antibiotic usage rate have significantly decreased, all to achieve the minimum standard of using the WHO recommendations for developing countries. The ratio of essential medicines after implementation of volatility between 32.12% and 43.33%, finances between 30.22% and 35.16%. Conclusion Rationality drug use factor have improved significantly. After the implementation of the essential medicine system, income structure of basic medical institutions factor also is gradually approach reasonable. The improvement of residents’ afford ability factor is lesser. This article suggests that the increased investment of policy should be considered in further, perfecting the related cost control measures and health monitoring indicator. |
