| 目的 DRGs视角下评价胃癌手术患者的住院服务综合绩效情况，为肿瘤专科服务持续优化作参考。方法 回顾搜集2014—2015年1 938份胃癌手术患者数据，对比DRGs分组情况、权重、平均住院日、次均费用、病例组合指数等指标，进行胃癌手术住院服务绩效分析。结果 胃癌手术收治病例基本集中在前5个DRGs组，2014年占比为88.99%，2015年占比为92.0%；不同术式的病例组合指数存在变动；胃肠中心收治病例的综合绩效高于非胃肠中心的综合绩效。结论 基于DRGs绩效评价分析，扩大病种收治力度，规范诊疗控制费用，专病专治、合理规划术式，有助于胃癌手术患者收治的绩效持续改进。 |

Objective
Through using DRGs tool to evaluate the comprehensive performance for gastric cancer inpatients，and provide references for continuous improvement for cancer treatment service. Methods 1 938 surgical cases were collected from 2014 to 2015，and performance indicators including the number of DRGs group，weight，case-mix index(CMI)，and length of stay and cost of per inpatient were utilized. Results Gastric cancer inpatients were mainly distributed in the first 5 DRGs groups，accounted for 88.9%，92.0% separately；CMI of different surgical procedures changed；The comprehensive performance of the gastrointestinal center was higher than that of the non-gastrointestinal center. Conclusion Based on the performance evaluation and analysis of DRGs-related indicators，increasing the number of disease treatment，and specializing the cost of diagnosis and treatment were recommended. Reasonable diagnosis and treatment and rational selection of operation contribute to the continuous improvement of medical service for gastric cancer surgical inpatients. |
