基于患者视角的公立医院补偿机制改革调查分析

摘要:
目的 调查上海市三级综合性医院门诊患者药品消费情况,探讨医事服务费的设立标准。方法 通过分层随机抽样的方法共选取320名患者进行问卷调查。对数据进行统计分析,并构建logistic回归模型。结果 患者购药负担较重,对医事服务费制度的接受程度良好;社区医院药品服务尚不能满足患者需求;年龄、收入和户籍属地是影响患者对医事服务费支持程度的主要因素等。结论 应转变补偿模式,试点医事服务收费,推动公立医院补偿机制的改革。

Analysis of the Investigation of on the Compensation Mechanism in Public Hospital Reform from Patients’ Perspective

Abstract:
Objective To investigate the pharmaceutical spending of patients in tertiary comprehensive hospitals in Shanghai,and to discuss the charging scale of medical service fees. Methods Using stratified-random sampling method according to the departments of hospitals,320 patients were selected to do the face-to-face survey. Descriptive analysis was conducted after the survey and a Logistic regression model was established. Results The economic burden for patients is heavy,and they have positive attitudes toward the medical service fees. Community hospitals cannot meet patients’ needs. What’s more,age,income,and non-local medical service are main factors that influence patients’ acceptable willingness of medical service fees. Conclusion Government should transform the compensation mechanism of public hospitals and pilot the medical service fee to push forward the medical reform.
