江苏省公立医院编制现状分析

摘要:
目的 分析江苏省省级和部分市县级公立医院人员及床位编制配置情况及存在问题,为公立医院编制制度改革提供参考依据。方法 采用典型抽样方法,对江苏省级(3家)、南京市级(11家)和句容市级(2家)所有公立医院开展问卷调查,采用描述性统计分析对定量数据进行分析,采用主题分析对定性访谈资料进行分析。结果 近4年间江苏省不同级别公立医院编制总量有所增加,但增速较缓;城市公立医院空编率有所提高,县级公立医院空编率逐年下降;公立医院编外人数和所占比例都不断增加,且医院级别越高,编外人员所占比重越高;公立医院编制床人比与标准床人比和实际床人比均差距较大。建议 修订公立医院编制标准,科学核定人员总量和规模;优化公立医院岗位结构,压缩行政后勤人员编制;控制总量,盘活存量,合理利用现有空编;探索实施编制审批制与备案制相结合的编制管理方式。

Status Analysis of the Public Hospitals’ Establishment in Jiangsu Province

Abstract:
Objective To analyze the status and problems of the allocation of staffs and beds establishment of public hospitals in Jiangsu Province to provide the references for the establishment system reform of public hospitals. Methods 3 provincial-level hospitals in Jiangsu Province,11 prefecture-level hospitals in Nanjing City and 2 county-level hospitals in Jurong City were selected to do questionnaire survey. Typical sampling method,descriptive analysis and thematic analysis were conducted with quantitative data and qualitative data. Results The amount of the staff establishment was increasing in public hospitals of different levels from 2012 to 2015 in Jiangsu Province,but the empty establishment amount is decreasing. The percentage of null establishment of city public hospital is increasing,but it is decreasing in county hospitals year by year. The amount and percentage of staffs without establishment in public hospitals are increasing gradually and the higher of the level of hospital,the proportion of the staffs without establishment is higher. There is a big gap between the ratio of establishment beds and establishment staffs, and the ratio of the actual beds and actual staffs. Conclusion The establishment standards of the public hospitals should be changed and the quantity, and scale of the staff be established scientifically. The structure of public hospitals’ positions should be optimized, and the establishments of administrative staffs be downsized. The total amount and activate the existing establishment should be controlled,and the null establishment be used reasonably. The new management method of combination of examination should be explored and implemented and system, and record keeping system be approved.
