| 通过调查乌鲁木齐市老年人的慢性病患病及健康认知情况，探讨目前在老年人慢性病健康管理过程中存在的问题。研究发现乌鲁木齐市老年人患慢性病情况较为严重，对慢性病相关健康知识掌握程度不容乐观。建议从健康赋权理论的角度出发，实现对老年人慢性病的健康管理。 |

| Through investigating the chronic disease of old people and health awareness in Urumqi,discussing the problems of chronic disease health management. The study found that older people suffering from chronic diseases in Urumqi is serious,The elderly’ chronic diseases related health knowledge level is not optimistic. It is suggested that the health of the elderly chronic disease management can be realized from the perspective of health empowerment theory. |
