支付方式改革是医疗控费、改善医疗费用结构、规范医师诊疗行为的有效途径。云南省禄丰县于2007年开始探索住院按床日付费改革，有效控制了药占比，体现医务人员劳动技术价值的费用占比也相应提升。但同时存在着住院费用项目间转移，检查、化验费用急剧增长等问题。透过住院费用结构变化情况，反映住院按床日付费在县级医院中的适用，提出规范医师诊疗行为，适当放开医疗服务定价权，推动医院内部绩效管理改革等建议。 |

| Payment reform is one of the effective ways of controlling the excessive growth of medical expenses, improving the structure of the medical expenses, and regulating the physician’s behavior. Lufeng County，Yunnan Province has explored the per diem payment in hospitalization since 2007 and effectively controlled the medicine proportion in the hospitalization expense，and the proportion of the cost that reflecting the medical staff technology value has improved. At the same time，transfer among projects cost of hospitalization and the rapid growth of test costs are very serious. The changes of hospitalization expenses structure reflect the application of per diem payment. Three advices are proposed, including the physician’s behavior，liberalizing the price of medical services，and promoting the performance management reform. |
