| 目的
总结分析精神专科医院实行收支两条线管理的成效，为完善上海市精神专科医院实行收支两条线管理机制和政策提供参考。方法 采用回顾性研究，对收集的上海市某区精神专科医院在实行收支两条线管理改革前（2009—2010年）与改革后（2011—2013年）以及与未实行收支两条线管理的上海市某两家精神专科医院2009—2013年度的有关调查资料进行比较分析。结果 实行收支两条线管理的精神专科医院较好地落实了基本医疗和公共卫生服务，服务数量明显提升，服务效率得到不同程度的提高，收支结构更趋合理，患者满意度稳步上升，虽然职工满意度有所下降，但绝大多数职工支持实行收支两条线管理。结论 建议政府进一步加大对精神卫生服务的投入力度，适时推广上海市精神专科医院实行收支两条线管理。 |

Objective The paper aims at analyzing and drawing a conclusion on management effectiveness of separating revenue from expenditures in psychiatric hospitals，which may be a reference for perfecting the mechanism and policy of separating revenue from expenditures in psychiatric hospitals in Shanghai.Methods A retrospective study was carried out to collect related data intending to conduct comparative analysis among a psychiatric hospital before（2009—2010） and after（2011—2013） putting separating revenue from expenditures management reform into practice in a region of Shanghai. Comparative analysis were also used between this hospital with other 2 hospitals which have not used separating revenue from expenditures management(2009—2013).Results Psychiatric hospitals executing separating revenue from expenditures management implement the basic medical and public health services much better and having gotten obvious service quantity improvement，as well as service efficiency in various degrees. Both the revenue and expenditure structure and patient satisfaction of those hospitals has become more reasonable and increased steadily and the vast majority of staff support separating revenue from expenditures, though employee satisfaction has experienced decrease to some extent.Conclusion It is recommended for the government to intensify investment in mental health services further and promote separating revenue from expenditures management in psychiatric hospitals in Shanghai properly. |
