目的
比较疾病诊断相关分组(diagnosis related groups,DRGs)相关指标与传统临床指标在医疗服务能力评价中的差异,分析DRGs在医疗服务能力评价中的可行性和可靠性,为医疗机构完善绩效考核模式、加强精细化管理提供参考。方法 以北京市某三级甲等医院S科室各主诊组2014年出院病案信息为资料,运用非整秩次RSR法对各主诊组在两类指标下的医疗服务能力进行综合评价及比较,并根据S科室KPI目标绩效考核结果检验DRGs指标评价结果的可靠性。结果 DRGs指标与传统指标的综合评价结果存在一定差异,KPI目标考核与主诊组DRGs评价结果有很强的一致性,一定程度上反映了DRGs在风险调整上的合理性和有效性。结论 随着我国DRGs的研究和应用的发展与成熟,主诊医师负责制和DRGs评价指标的结合能够为医务工作的过程管理和精细管理提供有效的支持与帮助。 |

Objectiv
e
Comparing the differences between DRGs-related and the traditional indexes in the evaluation of medical service capability, and analyzing the reliability and feasibility of DRGs-related indexes in the evaluation. Further, to provide the reference for improving the evaluation model and strengthening precision management of medical institutions. Methods Using RSR method to evaluate and compare the attending groups’ medical service capability under two kinds of indexes.Based on KPI target performance evaluation results, to verify the reliability of the evaluation results under DRGs-related indexes. Results There are some differences in the results of comprehensive evaluation between DRGs-related and traditional indexes. And KPI target assessment and attending groups’ evaluation results under DRGs-related indexes has a strong consistency, to some extent, reflecting the rationality and effectiveness of DRGs on risk-adjusted. Conclusion With the development and maturity of research and application of DRGs, combined with the attending in charge and evaluation DRGs-related indexes,can provide effective support and assistance for the process management and sophisticated management of medical work. |
