三级甲等公立医院院长对医院改革认知的调查

摘要:

目的 了解我国公立医院院长对医院改革相关政策的态度和意见。方法 对全国200家三级甲等公立医院院长进行分层随机抽样调查,共收集调查问卷178份。结果 研究发现,被调查者普遍认为院长不仅应具有人事处置权与经营自主权,而且应该有医院的建设发展决策权和结余分配权;院长更倾向于政府采取专项投入的方式来补助公立医院,认为调整医疗服务价格是补偿药品加成收入的最好方式。被调查者最关注医院的聘用制度,认为应该放宽或取消工资总额限制。结论 院长对所有人权利归属的认知与理论不相符,不利于医院管办职能分开;城市公立医院补偿机制改革应以医疗服务价格调整为主,重点保障医院正常运营;建立合理的医务人员薪酬分配制度,适当放宽工资总额控制。

Cognitive Survey of Public Hospital Reform Among Hospital Managers

Abstract:
Objective To understand the attitudes and opinions of Public Hospital managers for policies of hospital reform. Methods The stratified random sample survey were conducted in 200 public hospitals, and a total of 178 questionnaires were collected. Results The findings showed that respondents generally agreed that the managers should not only have the right to dispose of personnel and operational autonomy, but also should have investment decisions power and surplus distribution rights. Managers prefer the special subsidies from government, and considered the price adjustment policy is the best way to compensate for removal of drug markup. Finally, the respondents were most concerned about employment system that should relax or abolish the total wage constrains. Conclusion This paper provides several policy implications: the cognitive of ownership in president prevent the separation of ownership and management in public hospitals; the policy of price adjustment should be implemented in urban public hospitals; measures should be taken to establish the rational system of personnel and salary, and to relax the control of total wages.
