上海市第一人民医院住院病人地理分布的演变研究

摘要:
目的 描述医院住院病人的时空分布变化,并把该分布与医院发展态势结合起来等问题进行研究。方法 通过空间定位、空间分析、描述性统计与推断性统计的方法,分析上海市第一人民医院住院病人分布的时空变化。结果 1、从全国范围看,医院病人主要分布在东部地区,长三角地区病人数量最多,并向中部地区扩展;从上海市范围看,医院病人主要分布在本市中心城区及松江区,中心城区病人密度值逐步变小,宝山区、浦东新区及松江区的病人密度值则渐渐变大。2、从上海市范围看,北院病人主要分布在本市中心城区,并向东北地区扩展,虹口区病人密度值最高。南院病人主要分布在本市中心城区及松江区,并向西南地区扩展。南院病人数量增幅高于北部,两院区与住院病人的距离逐年扩大,南院与病人距离大于北院与病人的距离。讨论 病人地理分布影响因素包括:人口因素、医保政策、医院发展阶段、医院等级、区位条件等。GIS技术是研究病人地理分布及其演变的有效工具,能为医院管理者决策提供参考依据。

Abstract:

Objective To describe the time and space distribution of in-patients of a hospital, and combine the distribution with the hospital development. Methods Spatial orientation, spatial analysis, descriptive analysis and deductive analysis were used to make analysis of time and space evolution of in-patients of Shanghai First People’s Hospital. Results 1. On a national scale, the in-patients mainly distribute in the eastern provinces of China. The patient density has been growing up, of which the patient density of the Yangtze River Delta had been always the highest. From the scope of Shanghai, the in-patients of Shanghai First People’s Hospital mainly distributed in Songjiang District and the central area of Shanghai. While the density of patients of the central area declines, the density of patients in Baoshan District, Songjiang District and Pudong New Area rise up. 2. From the scope of Shanghai, the in-patients of the northern branch concentrate in the central area of Shanghai, and expand out of the central area, especially in the northeast regions. The density of patients in the Hongkou District has been the highest. The in-patients of the southern branch concentrate in the Songjiang District and central area of Shanghai and expand in the southwest areas. The growth rate of in-patients of the southern branch is higher than that of the northern branch. The distances between both the northern branch and southern branch and in-patients gradually enlarge. The distance between the southern branch and patients is farther than that between the northern branch and patients. Conclusion The factors influencing patient distribution include: population, health policy, development stage of hospital, hospital level and regional traffic conditions. GIS technology is an effective tool for understanding the possession and potential market of hospitals, and it will be great help to hospital managers to make decision.
