目的 调查本院门诊药房处方调配内差现状，分析其相关因素，针对各因素探讨防范对策，以降低门诊药房调配差错率，保证患者用药安全。方法 对本院2010年1—2月份门诊药房处方调配进行现场抽查，记录各种调配内差，对各类调配内差进行统计分析。结果 处方总量为
50 266张，存在调配内差的处方共377张，占总处方量的0.75% 。调配内差类型主要为数量差错（206张，占68.96%），品种差错（130张，占34.48%）以及规格差错（10张，占2.65%）；调配内差的发生率与工作人员资历、工作时间段几乎无关。结论 调配内差与药品包装、配药人员的工作态度相关，有必要探讨干预措施，提高处方配药的准确性，逐步实现门诊处方调配差错零宽容。 |

Objective
To investigate the prescription deployment errors in outpatient pharmacy of our hospital in the status quo, analyze the related factors and research the preventive measures against each factor in order to reduce the dispensing error rates and ensure the safety of medication. Methods Checking the prescription deployment in outpatient pharmacy randomly from January to February，2010, recording all kinds of deployment errors, and statistically analyzing various types. Results The total volume of prescriptions dispensed is 50 266, including 377 volume with dispending errors,taking up about 0.75% of total prescription volume. There are 3 main types of errors that are number error(260 volume, taking up about 68.96%), species errors(130 volume, taking up about 34.48%), and strengths errors(10 volume, taking up about 2.65%). The deployment errors occur in the staff with higher qualification are higher, which have little to do with the working time. Conclusion The deployment errors are related to the pharmaceutical packaging, attitude of the staff. Therefore, in order to prevent dispensing errors, it is very necessary to research the specific interventions to improve the accuracy of prescription deployment. |
